Passport Book or Card

Passport applications can be obtained and submitted through the University of Utah Learning Abroad Office. Application documents also available online.

  • An adult passport (16 years of age and older) is valid for 10 years.
  • A child’s passport (ages 0-15 years old) is valid for 5 years.
  • DO NOT sign the passport application until requested to do so.
  • If a name is different than the name on the birth certificate, you may be required to provide a legal document (such as a certified copy of a marriage license) to verify the name change.

Processing Times:

Refer to the US Department of State Website  for the most up to date processing times.

Proof of Citizenship:

  • If born within the United States you must provide a certified copy of your birth certificate. (ITEMS NOT ACCEPTABLE include: hospital certificate, baptism, and notification-of-birth and California abstracts of birth.)
  • If born outside of the United States you must provide a Certificate of Citizenship/Naturalization or report of birth abroad.

Personally Appear:

  • ALL APPLICANTS (including children) must personally appear when applying for a passport.
  • Children under 16 years of age must have both parents present

Applications for children under 16 require the presence and signature of both parents. If the second parent is not available to sign, the appearing parent must present evidence that he/she has:

  • sole legal custody of the child, or
  • notarized written consent of the other parent for the issuance of the passport, or
  • written statement explaining why non-applying parent's consent cannot be obtained.

Social security numbers must be provided for all passport applicants!

16 years or older:

Total $165.00 (two separate payments required)

  • $130.00 (payable to U.S. Department of State, check or money order only)
  • $35.00 (payable to The University of Utah - check, money order, MasterCard, Discover, AMEX, or Visa, No Cash Accepted.)

15 years or younger:

Total $135.00 (two separate payments required)

  • $100.00 (payable to U.S. Department of State, check or money order only)
  • $35.00 (payable to The University of Utah - check, money order, MasterCard, Discover, AMEX, or Visa, No Cash Accepted.)
All fees are regulated by the US Department of State - Bureau of Consular Affairs

Passport cards will facilitate entry at U.S. land and sea ports-of-entry when arriving from Canada, Mexico, the Caribbean and Bermuda.

The passport card may not be used to travel by air.

Passport Card Fees:

For first time applicants 16 years or older:

Total $65.00 (two separate payments required)

  • $30.00 (payable to U.S. Department of State, check or money order only)
  • $35.00 (payable to The University of Utah - check, money order, MasterCard, Discover, AMEX, or Visa, No Cash Accepted.)

For first time applicants 15 years or younger:

Total $50 (two separate payments required)

  • $15.00 (payable to U.S. Department of State, check or money order only)
  • $35.00 (payable to - The University of Utah - check, money order, MasterCard, Discover, AMEX, or Visa, No Cash Accepted.)
All fees are regulated by the US Department of State - Bureau of Consular Affairs

Expedited processing and mailing services are available:

  • Expedited Processing - $60.00 per application (payable to U.S. Department of State, check or money order only)
  • Overnight Delivery (passport book only) - $18.32 per application (payable to U.S. Department of State, check or money order only)

If you are traveling in less than 14 days (or 4 weeks if you need a visa), please contact the closest passport agency or center (proof of travel is required).

Passport Photo Fees:

 

Passport photos (set of 2): $12, includes tax

Tax Exempt: The purchaser must present a valid Utah State Exemption Certificate. (Utah Code Ann. 59-12-106(2))

  • Passport photographs are available for ages 5 years and older for a $12.00 fee. (check, money order, MasterCard, Discover, American Express, or Visa. No Cash Accepted.)
  • One U.S. passport photograph is required per application.
  • Passport photographs must have been taken in the last 6 months.
  • Pictures must be in color and on a white or light colored background.
  • Pictures must be two-inches square. (Image size of head must be between 1 inch and 1 3/8 inches including hair.) 
  • Eyeglasses, sunglasses, headphones or wireless hands-free devices are not allowed (See Department of State’s Passport Photos page for examples).
